Inside matters of musicians – bagpipes under the microscope.

The case of 61 years old man's death in 2014 received media coverage two years later. That's the time physicians needed to conclude the diagnosis: chronic hypersensitivity pneumonitis was caused by his instrument, precisely – fungi inside the bagpipes.

The man, struggling with dry cough and breathlessness for 7 years, has died in 2014 in Manchester (England) because of hypersensitivity pneumonitis – inflammatory disease triggered by chronc inhalation of various antigens from environment. This disease has gained many names regarding its cause, e.g. “farmer's lung” or “bird fancier’s lung”. Reports from recent years link this disorder to completely new environment – wind musicians. The inside of an instrument – either woodwind or brass – because of constantly blowing air and moistureness, is a suitable environment for growing microbial pathogens. Cases of severe lung diseases, caused by not properly cleaned instruments, previously included saxophonist, trombonist and bagpiper.

Patient from Manchaster unfortunaltelly did not recover. From the bag of his pipes microbiologists isolated fungi Paecilomyces variotti, Fusarium oxysporum, Rhodotorula mucilaginosa and Penicillium sp. The mold was visible on a mouthpiece, inside pipes and in the bag. The first such case of death was commemorated by the new name “bagpipe lung”. Authors of the publication point that examinig patient's habits and hobby is very important in the case of diseases of unknown etiology. Here, the primary cause of disease was identified two years after patient's death.

Desrcibed report received media coverage a couple of weeks ago, bringing a warning for wind musicians against microbes colonizing the inside of their instruments. With no doubt, a proper hygiene during work is of great importance in this case. However, Konrad Waleski, the bagpiper from Threecity, Poland, wants to chill the atmosphere – Bagpiper who died has playing practically the museum exibit – tanned animal skin sealed with beeswax etc. (…) Nowadays, bags in scottish bagpipes are made from Gore-Tex, or at least upholstered with Gore-Tex inside if we decide to use animal skin. In addition, such bag is always equipped with a special zipper allowing to open and properely clean the instrument inside. Currently, impossibility to thoroughly care for an instrument is not an exuse ^[source].
