Zika virus – Anders Widell’s overview

Like you can see, we are making series of articles from Vilnius International Summit on Communicable Diseases (26 June-1 July 2016) and the next part of it is an interview with retired virologist, Anders Widell (Lund University), who had there a presentation about Zika virus. We decided to focus on that topic mainly because last time media showed several cases of infections also in Europe, which is not theoretically defined as a danger zone.

Theoretically, because agressive mosquitos aedes aegypti i aedes albopictus, which transmit the virus, lives in tropical climate, like in South and Central America, South and East Asia or Pacific regions. Emerging viral epidemics from animals are unpredictable but still often due to changed habitat of vectors, which in turn may be due to deforestation, travel and climate changes. For example, mainly because of global warming, these mosquitos are now moving north from typical subtropical regions to Mediterranean regions and United States, even to Long Island and New York.

Let’s see, how occurrence of this virus developed. Although first identified in a febrile monkey in the Zika forest in 1947, ZIKV left little epidemiological imprint in the human disease panorama for 50 years. Then it was found in several countries of tropical Asia and Africa. In 2007 Zika unexpectedly infected 75% of Micronesia Yap islanders by aedes mosquito bites. In 2013 ZIKV was found in epidemics spreading east with aedes to many Pacific island nations including French Polynesia, Cook Island and Easter Island. In 2015 Zika epidemic arrived in South America, infecting 1.5 million Brazilians and spreading throughout Latin America – and then to Cap Verde Islands.

Symptoms also developed in very negative way. At the beginning, the world of science and business were not interested in making vaccinate agianst Zika, because symptoms were mild with maculopapular rash, low degree fever, conjunctivitis, joint and muscle pain. The infection could often pass without symptoms. Nowadays, Zika is not considered bengin anymore because of 2 severe complications, which occur in ZIKV endemic countries. These two are microcephaly and/or other neurological damage in fetuses/newborns and Guilain-Barré Syndrome (GBS) in adults. That is way such big companies like Glaxo SmithCline, Sanofi Pasteur, Inovio Pharmaceuticals Inc. are trying to discover a vaccinate and that is way World Health Organization (WHO) named Zika Virus global danger for public health, equally harmful as Ebola.

Also we, Europeans, should be very cautious beacuse the danger occur when we travel to tropical regions and than have careless sex. Why? Beacuase, contrary to popular belief, Zika Virus is transmitted not only by mosquitos but also sexually. Experts definitely discourage pregnent women and women who are planning to become mother in the nearest future to visit high-risk regions till the situation is not fully controled.

Data are still incomplete but if a pregnant women during trimesters I and II is infected by ZIKV there is a 1-2% risk that maternal viremia passes placenta and infects the fetal brain and causes microcephaly and/or brain damage like tissue loss, calcifications, enlarged ventricles, agenesis of corpus callosum end cerebellum. These very severe sequelae have been observed following the outbreaks in both French Polynesia and Brazil and can be interpreted as ZIKV invasion of growing fetal neural cells. GBS in adults is thought to be immunological. Diagnosis is based ZIKV RNA detection in blood, urine and saliva by PCR during the first 7-10 days, thereafter shifting to specific IgM and IgG serological tests. No vaccine is available, but a recombinant Yellow Fever/ZIKV vaccine is explored – this principle has proven successful for dengue vaccine. We have to believe that it will also become effective for Zika virus, but we should remember that making vaccine normally takes 10 years. If you believe what scientists from National Institutes of Health (NIH) said, first two vaccines will be tested by the end of 2016.
